Precision medicine is transforming healthcare from a population-based approach to one that considers genetic, molecular, environmental, lifestyle, and clinical differences. It aims to develop preventive, diagnostic, and therapeutic treatments that are suitable for physiologically significant patient populations. Genomics, transcriptomics, proteomics, metabolomics, pharmacogenomics, AI, nanotechnology, biotechnology, and digital health are all accelerating this transition. Pharmaceutical sciences are vital because precision medicine requires targeted pharmaceuticals, biomarker guided therapies, biologics, gene and cell therapies, RNA therapeutics, and personalized drug delivery systems. Pharmacogenomics can help predict variation in efficacy and toxicity, whilst companion diagnostics help select patients who are likely to benefit from specific medications. Artificial intelligence and machine learning are increasingly being used in target selection, molecular design, virtual screening, pharmacokinetic/pharmacodynamic prediction, biomarker discovery, and clinical trial optimization. Nanomedicine could improve target selection, bioavailability, controlled release, and therapeutic index. The primary obstacles include data quality, privacy, algorithmic bias, regulatory science, cost, clinical validation, infrastructure, and equal access.
