FREQUENCY OF MYOCARDIAL INFARCTION IN NON DIALYSIS CHRONIC KIDNEY DISEASE PATIENTS

Objective: To determine the frequency of myocardial infarction in non-dialysis chronic kidney disease patients. Study design: cross-sectional study. Setting: This study was performed in Nephrology Department Jinnah Postgraduate Medical Centre (JPMC) in Karachi Period: For six months duration from July 2025 – December 2025. Methods: The study started following approval by CPSP IRB/REU. Non-probability consecutive sampling was used to recruit a total of 84 patients (18-60 years) with diagnosed CKD (eGFR <60 mL/min/1.73 m² for ≥3 months) and who were not on dialysis. MI was diagnosed based on clinical symptoms, electrocardiographic changes, and/or troponin I ≥1.0 ng/ml. The data collected were analysed using SPSS version 25. For qualitative variables, frequencies and percentages were calculated and stratification was done using Chi-square test. Results: The majority of participants were in the age group 46-60 years and just slightly more male than female. Hypertension (69.0%) and diabetes mellitus (52.4%) were the most common comorbidities. Most patients were in CKD Stage 3B (31.0%) and Stage 4 (28.6%). The total incidence of myocardial infarction was 54.8% (n=46). MI increased progressively with increasing CKD stage with the highest being present in the Stage 3B (69.2%) and the next being in Stage 4 (66.7%). Conclusions: MI is a common event in non-dialysis CKD and is exacerbated by progressive CKD stage. In this high-risk population, early cardiovascular risk assessment and early interventions are essential.
