Total joint arthroplasty has become one of the most successful orthopedic interventions for relieving pain, restoring joint function, and improving the quality of life of patients with advanced hip and knee disorders. This review synthesizes recent evidence on advances in arthroplasty, focusing on surgical innovations, implant developments, perioperative management, rehabilitation strategies, and the diagnosis and management of periprosthetic joint infection. The review highlights the growing role of robotic-assisted surgery, biomarker-based diagnostic techniques, microbiological profiling, and evidence-based clinical pathways in enhancing surgical precision, early complication detection, and long-term implant performance. It also examines the factors influencing postoperative outcomes, including patient selection, multidisciplinary care, and technological integration, while discussing current challenges related to infection, implant longevity, and clinical decision-making. Recent developments demonstrate that combining precision surgery with improved diagnostic approaches and personalized treatment strategies contributes to safer procedures, reduced complications, and better functional recovery. Overall, contemporary arthroplasty practice continues to evolve toward patient-centered, technology-driven care that supports durable clinical outcomes and sustainable improvements in musculoskeletal health. The findings provide a comprehensive overview of current evidence and identify key considerations for optimizing future arthroplasty research and clinical practice.
