Abstract

The integration of medicine and pharmaceuticals is reshaping healthcare delivery by linking clinical decision-making, pharmaceutical innovation, digital health, and patient-centered care. This comprehensive review examines how integrated medicine–pharmaceutical care contributes to improved treatment outcomes through coordinated service delivery, precision therapeutics, digital technologies, and multidisciplinary collaboration. The review highlights the evolution from drug-centered and disease-centered models toward patient-centered, value-based systems in which pharmaceutical care supports diagnosis, medication optimization, therapeutic monitoring, adherence, and safety. Key innovations, including pharmacogenomics, biologics, biosimilars, cell and gene therapies, targeted drug delivery, artificial intelligence, digital therapeutics, and nanomedicine, are discussed in relation to their impact on personalized and efficient healthcare. The integration of electronic health records, telepharmacy, remote patient monitoring, mobile health, and big data analytics further strengthens clinical decision support and continuous care. Integrated healthcare improves outcomes by enhancing adherence, patient engagement, clinical effectiveness, pharmacovigilance, equity, and cost-effectiveness. However, regulatory complexity, data privacy risks, interoperability barriers, unequal access, and implementation limitations remain major challenges. Future progress will depend on ethical governance, secure digital infrastructure, inclusive innovation, and stronger collaboration among healthcare professionals, researchers, policymakers, and pharmaceutical stakeholders. Overall, integrated medicine–pharmaceutical care offers a promising pathway toward safer, more personalized, accessible, and outcome-driven healthcare systems.
